Post Acute Care Long-term Care Advanced Practice Provider (Jackson, MI)

The Certified Nurse Practitioner (Nurse Practitioner), in collaboration with physicians, Care Coordinators, staff RNs, and other healthcare professionals, provides holistic, compassionate care to post-acute care facilities. The Nurse Practitioner practices within a collaborative management context, diagnosing, managing, and preventing acute and chronic illness, and promoting wellness. They work hours as needed.

Requirements

• Perform in-depth health risk assessments on assigned patients
• Collaborate with physicians, Care Coordinators, facility staff, and family members
• Interpret medical and diagnostic data and recommend treatment plans
• Write and interpret lab results
• Maintain a focus on customer service and timely communication
• Meet deadlines and utilize time effectively
