Case Manager

This Role of Case Manager is responsible for managing individual service plans, coordinating clinical support, reporting, and supervision of front-line staff. The role involves comprehensive reports, data analysis, collaboration with other service providers, and ensuring adherence to Innovative Day’s policies and regulations. Case Managers will also contribute to staff development and quality assurance initiatives.

Requirements

• At least a bachelor’s degree from an accredited college or university, in a human service, medical, social services, education or clinical services
• At minimum one year’s experience in direct service provision to people with ID/DD
• Excellent Communication and Documentation skills, proficiency using Microsoft Office
• Experience in providing training to staff
• Experience in working with and coordinating care in an interdisciplinary team
