Job Description
The Case Manager is responsible for providing case management and housing stabilization services to homeless Veteran families. The position involves coordinating intake processes, conducting home visits, and linking clients to supportive services.
Requirements
- High school diploma required, Bachelor's degree in Social Services or related field a plus
- 1 year experience providing direct support services to individuals experiencing homelessness, mental health, substance abuse or other social issues
- Proficient in use of computer and Microsoft office suite (Word, excel, outlook and TEAMS)
- Valid California driver license, reliable personal vehicle, and current personal auto insurance required
