Job Description
The LVN Case Manager (CM) at this location provides support to high-cost, high-need members through systematic, community-based care management. This role focuses on person-centered care, collaboration with healthcare providers, and working with members in both clinic and community settings, often providing case management support, health education, and connecting them with resources. The CM supports members managing chronic health conditions, substance abuse issues, and facing behavioral or institutional barriers.
Requirements
- Engage with patients in-person and on the phone.
