Licensed Practical Nurse - LPN

First Choice Home Health is seeking full-time or part-time LPN/LVN to provide community-based care to patients in Boise, Idaho and the Nampa, ID metro area. This role involves coordinating patient care, administering medications, and educating patients and families. The LPN will work under the supervision of a registered nurse and a shared, multidisciplinary team.

Requirements

• Provide nursing care to patients of all ages in their homes.
• Coordinate care with the interdisciplinary group, patient/family, and referring agency.
• Assist in the development and implementation of patient care plans.
• Monitor patient health and report any changes to the supervising RN.
• Administer medications and treatments as prescribed.
• Educate patients and their families on health care needs, conditions, and options.
• Maintain accurate and timely documentation of patient care and progress.
• Reliable transportation and valid auto liability insurance.
