Job Description
Hospice de la Luz is seeking LPNs to provide community-based care for patients with home health services in the Santa Fe, NM region. The role involves providing nursing care under supervision, coordinating care with interdisciplinary teams, and ensuring patient outcomes. The position focuses on providing patient-centered care in a home environment.
Requirements
- Graduate of an accredited School of Vocational/Practical Nursing.
- Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC).
- Must have a current driver's license.
- Experience as a Licensed Vocational/Practical Nurse in a clinical care setting.
- Home health experience highly preferred.
