Physical Therapist – L’Chaim Adult Day Health Care

Jewish Family and Children's Services (JFCS) is seeking a part-time Physical Therapist to plan, implement, and manage physical therapy services for Russian-speaking older adults and individuals with disabilities. This role provides individualized treatment plans, restorative therapy, and support for achieving independence and well-being. Requirements

• Conduct initial physical therapy assessments and reassessments.
• Design and oversee restorative and maintenance therapy programs.
• Provide or supervise therapy interventions.
• Document assessments, treatments, care plans, and progress notes.
• Monitor participant condition and ensure safety.
• Collaborate with interdisciplinary teams.
• Communicate effectively with participants and caregivers.
• Participate in multidisciplinary team meetings.
• Contribute to outpatient physical therapy services
