Requirements
- Bachelor's degree preferred
- Community Health Worker certification required
- At least 2 years of supervisory experience in non-profit, healthcare, or community based organization
- Demonstrated experience conducting field-based outreach, case management, or direct community engagement
- Bilingual in English and Spanish required
- Strong written and verbal communication skills
- Ability to build and maintain strong relationships with community partners
- Ability to work occasional evenings or weekends for community events
- Proficiency with electronic health record systems like Athena Health or eClinicalWorks
