Population Health Manager - Registered Nurse

Kaweah Health is seeking a Population Health Manager - Registered Nurse to lead and direct assigned staff to provide expert support to Population Health project teams and relevant departments. The ideal candidate will have a strong background in project management, data analysis, and population health, with experience in Medicare Population Health Management efforts. The Population Health Manager will be responsible for supporting, coordinating, and facilitating the development of short and long-term work plans, monitoring progress towards goals, and evaluating performance of Population Health projects.
