Population Health Program Manager

We are seeking a Population Health Program Manager to lead the development, implementation, and performance management of population health initiatives aligned with organizational strategy. The role requires collaboration with clinical, operational, and analytics leaders to drive care transformation, support physician engagement, and implement programs that improve quality, outcomes, and value across the continuum of care.

Requirements

• Bachelor’s degree in Population Health, Clinical Quality/Clinical Process Improvement, or Nursing
• Current, valid AZ RN License
• 3 years of population health, quality/process improvement, nursing leadership, or project management
• Excellent communication and presentation skills
• Effective in working with all levels of clinicians and staff
• Demonstrated project management and organizational skills
• Robust analytics and problem-solving skills
