Social Worker

The Social Worker maintains the social and material needs of residents in a senior living community, acting as a liaison between residents, family, and community. They must have good organizational skills and ability to communicate effectively.

Requirements

• Know facility policies and procedures and fire/disaster plan
• Attend all required in services and meetings
• Maintain communication with all disciplines so each resident’s plan of care is comprehensive
• Maintain firm adherence to the Resident Bill of Rights and strict confidentiality concerning all aspects of resident’s care
• Conduct psychosocial assessment/social history
• Document resident psychosocial status initial and ongoing team notes, goals and progress
• Coordination of resident discharge planning and follow up with resident, family, interdisciplinary team and community services
• Review, educate and assist in completion of advance directives with resident and or designated representative
• Assist with end of life care planning
• Participate in resident and family council as requested
