Job Description
Responsibilities as a Service Coordinator include assessment and verification of service coordination needs, service planning, referral, and monitoring of service provision, as well as after-hours crisis services and documentation of progress. Frequent contact with consumers, family members, and service providers is required, with a flexible schedule including after-hours, holidays, and weekends.
Requirements
- Bachelor's or advanced degree from an accredited college or university in a social, behavioral, human service, or health-related field
- Valid Texas driver's license and an acceptable driving record
- Personal automobile liability insurance as required by the state of Texas
