Job Description
We are seeking a RN Case Manager to join our dedicated team at South Port Square in Port Charlotte, FL. The successful candidate will be responsible for ensuring a successful move-in of all future residents and performing daily responsibilities such as resident evaluations, move-in process, and discharge planning. The role requires a RN license in good standing, at least 6 months nursing experience, and excellent communication skills.
Requirements
- RN currently licensed by the State of Florida in good standing
- At least 6 months nursing experience. Geriatric involvement preferable
- Can generate a warm, friendly, and caring manner on first impression
- Can communicate effectively with residents, families, staff, and visitors
- Maintain the highest level of confidentiality
- Can pass a pre-employment drug screen and criminal background check
