We are seeking a Health and Social Care Coordinator to provide an excellent customer focused service to Citizens, Carers, and other colleagues and partners requesting Adult Social Care or health services. The role involves assessing and triaging needs, liaising with social care and health professionals, and signposting to appropriate provision and services.
Requirements
- Excellent customer service skills and the ability to work in a busy customer contact centre
- Ability to communicate effectively with vulnerable Citizens and Carers and their family, demonstrating excellent communication and listening skills
- Extensive experience of using the MS office suite
- Ability to undertake high level office administrative duties
- Ability to understand and deal with confidential and sensitive issues when liaising with both team members and other professionals
