Hospice Volunteer Heart of Hospice

The volunteer plays a vital role in enhancing the quality of care delivered to the patient and family through patient/family care activities or administrative activities. This is a volunteer position and ineligible for pay, focusing on activities such as providing assistance in the hospice program, administering the Plan of care, and documentation of patient encounters.

Requirements

• Provides assistance to the hospice program and/or patient/family in accordance with designated activities.
• Provides input into the Plan of care and Interdisciplinary Group meetings.
• Accurately documents services provided.
• Maintains strict patient confidentiality.
• Attends at least four (4) in-services per year.
• Participates in community outreach projects.
• Bereavement Volunteer: knowledge of the death and dying process.
• Patient Care Volunteer: records accurate documentation of encounters.
• Administrative Volunteer: completes assignments on time.
