Job Description
The Substance Use Disorder (SUD) Case Manager is responsible for the overall coordination of care and supporting clients/clients through their recovery journey.
Requirements
- Work cohesively with the Clinical Outreach Team to coordinate transitioning levels of care.
- Participate actively in interdisciplinary treatment team meetings, providing input on client progress and care needs.
- Develops, implements, manages, and continually evaluates individualized client care plans and discharge plans.
