Job Description
As a Case Manager, Psychogeriatric, you will co-ordinate a support network and linkages for older adult and seniors with serious mental illness and other challenges, and provide direct service and support to clients in daily living, practical assistance, crisis intervention, and linkages to primary care and other services.
Requirements
- Completion of degree in social services/human services or equivalent
- Minimum 3 years of experience in providing support to vulnerable seniors and older adults with special needs
- Group counselling related to mental health, addictions, and related issues
- Knowledge of community resources and support
- Valid drivers’ license, ability to drive work vehicles to transport clients
