Care Coordinator

Lutheran Community Services NW is a non-profit organization that serves communities throughout Washington, Oregon, and Idaho with a wide range of services for individuals of all ages and backgrounds. The Care Coordinator position works with youth and families enrolled in the WISe program, facilitating the Child and Family Team (CFT) process and supporting individualized care.

Requirements

• Bachelor’s degree in social work, psychology, or a related field
• One year of related experience in behavioral health, social work, psychology, or a related field
• Fluency in English, both spoken and written
• Intermediate proficiency in business applications and productivity suites
• Counselor Agency Affiliated within 60 days of hire
