Job Description
The Salvation Army is seeking a Case Manager to provide direct supervision and case management services to transitional aged youth experiencing homelessness, with the goal of helping participants transition to permanent housing.
Requirements
- Provide individualized case management, advocacy, and referral services to transitional aged youth
- Support individuals in developing independent living skills and securing permanent housing
- Maintain documentation in accordance with regulatory and internal policies
- Engage clients in individual case management sessions on a weekly basis
- Provide referrals for group and/or individual mental health counseling and other services
