Job Description
This position serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders, assisting them to live as independently as possible in their chosen community. The case manager will work under direct supervision and will be responsible for providing direct services to clients, including transportation to appointments and groups. The position requires a bachelor's degree in Social Services or Human Services or equivalent experience, and a valid driver's license with personal vehicle insurance.
Requirements
- Bachelor’s degree in Social Services or Human Services and (1) year of experience or High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
