Case Manager III - Mental Health 639

Serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders, assisting them to live independently in their chosen community.

Requirements

• Bachelor’s degree in Social Services or Human Services
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
• High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services
