LSW - Licensed Social Worker

Maine Veterans’ Homes is seeking a Licensed Social Worker (LSW) to provide psychosocial wellness to veterans, spouses, and Gold Star Parents at their Caribou, ME location. The role involves assessing residents, facilitating discharge planning, and collaborating with other healthcare professionals. The ideal candidate will be able to communicate effectively with residents and other members of the healthcare team.

Requirements

• Current and valid Social Work License from the State of Maine
• Bachelor's Degree
• Long-term care experience (preferred)
• Knowledge of VA, Medicare & MaineCare guidelines (preferred)
• Experience with case management, care planning and discharge planning (preferred)
• Proficiency in spoken and written English
• Basic computer skills
