Job Description
Inpatient Behavioral Health Case Manager responsible for providing comprehensive care management services to patients, coordinating services, and promoting positive patient outcomes in an inpatient psychiatric setting.
Requirements
- Experience in behavioral health, care management, or related clinical setting
- Maine State license as a Licensed Social Worker (LSW OR LSX), Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Registered Nurse (RN)
- Strong assessment, communication, and care coordination skills
- Ability to work collaboratively within a multidisciplinary team
- Patient-centered, compassionate approach to care management
