Case Manager (HFA Advocate) supporting families through strength-based, trauma-informed home visiting services in Bucks, Lehigh, Montgomery, and Philadelphia counties.
Requirements
- Associates, Bachelor's or Master's degree in Social Work, Public Health or related field.
- Valid driver's license and clean driving record.
- Experience with case management, community health, or supporting families with complex needs.
- Strong documentation and organizational skills.
- Comfortable working independently
