Medical Care Coordinator

The Medical Care Coordinator will collaborate with medical providers and nursing staff to support patients with social and clinical needs, primarily through coordination of care, transportation, and patient scheduling. This role focuses on improving patient health and navigating referrals and supporting patients in optimizing medication adherence. The position requires a blend of clinical duties, administrative tasks, and an ability to thrive in a fast-paced healthcare environment.

Requirements

• Completed Medical Assistant training in accredited program.
• 1-2 years of experience in social services or equivalent skills.
• Current Cardio-Pulmonary Resuscitation (CPR) certification.
• Flexible and organized
• Proficiency in Microsoft Office (Excel, Office 365).
• Strong communication and interpersonal skills.
• Bilingual fluency in Haitian-Creole or Spanish (preferred).
• 3+ years of experience working with diverse populations in a healthcare setting (preferred).
• Advanced computer skills (EPIC OCHIN a plus).
