Health Services Coordinator (LPN)

MBK Senior Living is seeking a part-time Health Services Coordinator (LPN) to join their team at the Sky Vista community in Mesa, AZ. The role involves supporting the overall operations of the health services department and providing care and services to residents according to individual service plans.

Requirements

• Conduct and coordinate assessments/evaluations of potential residents and make recommendations for admission
• Draft initial individualized Service Plans and update as needed
• Review service plans with responsible parties and Executive Director as requested by Director of Health Services
• Provide training and education on essential topics such as medication delivery, safety protocols, and emergency procedures
• Perform all assigned duties accurately and timely, including required documentation
• Serve as a medication technician, if needed
• Provide coverage of job duties within the department during absences
