Job Description
The Unaccompanied Children LTFC Case Manager is responsible for coordinating case management services for children in care, ensuring consistent care coordination communication between medical and behavioral health staff, and providing direct assistance to sponsors for timely release of youth. This includes creating required reports, facilitating case management meetings, and ensuring effective working relationships with families, children/youth, and colleagues.
Requirements
- Bilingual
- Child Welfare experience
- Bachelor's degree in behavioral sciences, human services or social services field
- 2 years' experience in the fields
- Valid driver's license
- CPR/First Aid certification
- Ability to obtain federal fingerprinting background checks
