Social Worker (Part-Time)

Medcan is seeking a part-time Social Worker to join their team in Oakville. The ideal candidate will have a Master's degree in counselling, psychology or social work, and 3+ years of clinical experience with adults.

Requirements

• Reviewing and interpreting client screening results
• Providing recommendations or referrals for ongoing treatment
• Leveraging Medcan’s Preferred Provider Network
• Developing a client action plan
• Building strong relationships with the client's Annual Health Assessment team
• Creating a summary report to reflect relevant client insights and action plans
• Supporting new program development
• Updating client resources and clinical tools
• Evaluating and reporting on external providers and community resources
