Job Description
We are seeking a dedicated CoCM Behavioral Health Care Manager to join our team. The role involves reviewing patient charts, communicating with the Clinical department and providers, and providing assessment and care management services.
Requirements
- Bachelor's degree in social work or a Registered Nurse (RN) license
- Experience in long-term care preferred
- Experience in behavioral health preferred
