Behavioral Health Care Manager

Optimal Care is a clinician-owned and operated company that provides physician services, home health, and hospice care. We are looking for a Behavioral Health Care Manager to support and coordinate the mental and physical health care of patients on an assigned patient caseload.

Requirements

• Bachelor's degree from an accredited school in Social Work, Nursing, or Psychology
• Minimum of 2 years of social work or care management experience
• Current and valid driver's license
• Reliable transportation and valid automobile insurance coverage
