Job Description
Infinite Medical P.C. is seeking a Dialysis Care Practitioner NP/PA to provide on-site care to long term care residents in a dialysis center. The role involves clinical management, patient monitoring, interdisciplinary coordination, and educating patients and families. They prioritize compassionate, personalized care and innovative collaboration.
Requirements
- Active and unrestricted Nurse Practitioner, Physician Assistant license
- State-specific license in good standing in Louisiana
- Minimum 2 years of experience as a dialysis NP/PA
- Willingness to work with dialysis patients in skilled nursing and long term care facilities
- Strong communication, clinical, and critical thinking skills
- Passion for elder care and person-centered healthcare
- Ability to lift a minimum of 20 pounds
- Must be able to stand / walk / work on your feet up to 6-8 hours per day
