Job Description
Under the direction of the Medical Director and Co-Medical Director, the Nurse Practitioner (NP) ensures the healthcare of incarcerated patients undergoing dialysis treatment within a correctional facility.
Requirements
- Manage a clinical caseload of ESRD and pre-ESRD patients
- Perform accurate comprehensive history-taking and diagnosis
- Prescribe safe and appropriate medications
- Exhibit excellent communication and written skills
- Monitor dialysis patients in partnership with nephrologists and facility staff
- Educate patients on disease processes and dialysis management
- Collaborate with other healthcare providers
- Refer patients to external services or specialists
- Contribute to the development of new services and patient education programs
- Participate in quality improvement initiatives
