Case Manager

MHA is seeking a Case Manager to join our Assisted Outpatient Treatment Services Team operating out of Morris County. As a Case Manager, you will provide on-going monitoring of individuals in the AOT program, conduct intake assessments, and develop treatment plans. You will also provide linkage and service coordination to individuals, assist with case management needs, and document observations and services in progress notes.

Requirements

• Master's degree in social work, psychology, counseling or a related field
• Experience working with individuals with severe and persistent mental illness
• Able to work a flexible schedule, evenings, weekend and holiday hours
• Valid driver’s license in good standing
