Meridian Home Health seeks exceptional, qualified Licensed Vocational Nurses to service its homebound Home Health patients in the Bay Area and the surrounding communities.

Requirements

• Provide direct patient care as initiated by the registered nurse
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
• Report to supervisor on all patients under your care
• Provide accurate and timely documentation
• Participates in coordination of home health services with team
• Performs other duties as assigned by the registered nurse
