Job Description
Under the supervision of the Area Director, the PCCM shall be responsible for person-centered, strengths-based case management services for individuals in SUD and MH treatment programs.
Requirements
- High school graduate or equivalent
- Certified by the Kansas Department of Aging and Disability Services as a Person-Centered Case Manager
- Ability to navigate EHR
- Minimum of one-year paid experience providing case management or workforce development services to a similar population
- Excellent interpersonal skills and ability to communicate effectively
- Ability to maintain a team spirit and work cooperatively with others
- Must be adaptable to change and have effective organizational skills
- Must be able to organize and follow through on multiple tasks
- Display consistent, good judgment and decision-making skills
- Ability to establish and maintain rapport with target population
- Must be able to pass background/security check
