Case Manager

The Case Manager is responsible for coordinating case management services, linking clients to community-based resources, and providing education and support to help young adults maintain stable housing. The role involves developing individualized support plans and fostering connections to essential services.

Requirements

• Bachelor's degree in human services preferred
• Two years working in human services including experience working with young adults
• One year's experience in case management
• Valid and insurable driver's license
• Ability to organize and have clear communication both orally and in writing
• Excellent organizational skills, problem-solving skills, ability to be flexible, and the ability to follow through while multiple tasking
