Job Description
The Acute Care Services / Mobile Crisis Clinician will provide 24/7 mobile outreach to individuals in psychiatric crisis, working in partnership with police, first responders, schools, and other community stakeholders. The role involves rapid evaluation, crisis intervention, referrals to aftercare, and facilitating discharge. This position is a vital component of a team focused on preventing hospitalizations and supporting clients and families.
Requirements
- Rapid and accurately evaluate complex behavioral health situations
- Develop and recommend crisis interventions
- Facilitate voluntary inpatient/emergency/discharge revocation
- Provide crisis stabilization follow-up
- Refer to aftercare agencies
- Preventive and supportive interventions
- Collaborative planning and implementation
- Utilize Crisis Prevention Plans and Client Alerts
- Provide provisional treatment planning
- Travel within and outside the Community Mental Health Center area (50-75%)
