Job Description
A Case Manager coordinates therapeutic, educational, and basic support services for psychiatrically disabled individuals in a least restrictive environment. They provide direct intervention in the community and direct care to ensure success in maintenance and continuity of care.
Requirements
- BA in a human service field
- Skills in advocacy, outreach, assessment, crisis intervention and supportive counseling
- Strong communication skills
