Job Description
The Mental Health Case Manager is responsible for providing direct support to clients and their families enrolled in Outpatient MH services, including accessing services, outreach/engagement support, service coordination/referrals, case management, and discharge planning.
Requirements
- A Bachelor’s degree in Behavioral Health or related field
- Care Coordination or Case Management experience at a behavioral health or related agency
- Knowledge of state and county systems and resources
- Bilingual abilities, including the ability to speak, read, write, and translate documents in Spanish
- QMHA-Registration within 30 days of hire
- Personal transportation allowing for travel between program sites
