Mountain View Hospital is looking for a Discharge Planner to join their team to work in close collaboration with physician healthcare personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patients' health problems. Prepares patient and family for safe discharge from acute care hospital setting.
Requirements
- Coordinate the discharge plan and services with the patient, their families and disciplinary teams
- Work with patients to obtain maximum benefits from medical care
- Review and validate physician orders, report progress and unusual occurrences on patients
- Ensure cost effective and appropriate health care services are provided to all patients
- Collaborate with other departments in discharge planning
- Assess the physical, functional, social, psychological, environmental and financial needs of patients
- Assist with referrals to appropriate community resources
- Act as a patient advocate by assisting in acquiring appropriate medical, nursing or other health care in the home or clinic setting
- Maintain accurate patient records, ensuring patient confidentiality
- Contribute and assist in the performance of case management activities
