Job Description
The Behavioral Health Provider will work as part of the primary care treatment team to identify, triage, and manage patients with medical and behavioral health needs. The provider will also provide skill training, develop behavioral change plans, and coordinate care with other providers and staff.
Requirements
- Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) in the state of Oregon
- Ability to respond to crisis situations
- Experience working in a medical system and with individuals with complex medical and mental health issues
- Knowledge of the local behavioral health treatment system and community resources
- Professional demeanor and interpersonal communication skills
