Service Coordinator I will coordinate programs and services to help participants maintain a good quality of life and age in place. The Service Coordinator prioritizes a person-centric model of service delivery.

Requirements

• Coordinate programs and activities for eligible individuals
• Serve as a liaison to community agencies and network with service providers
• Monitor the provision of services and document participant use
• Develop and maintain a Resource Directory of service providers
• Organize educational events and promote participation
• Assist participants in building informal support networks
