Family Practice Physician

Native Health is seeking a Family Practice Physician to provide comprehensive primary patient care services, including prevention, health maintenance, diagnosis, treatment of acute and chronic conditions, and follow-up services to patients. The ideal candidate will have strong knowledge of current evidence-based family medicine guidelines and excellent communication skills.

Requirements

• Completion of an accredited medical school
• Permanent and unrestricted license to practice medicine or osteopathy in Arizona
• Current board certification in family practice
• Current medication dispensing license (DEA)
• Three years of experience in an outpatient family practice or comparable setting
• Current BLS certification
• Current ACLS certification (preferred)
• Valid and unrestricted Arizona driver’s license
• Pass a criminal background check and obtain a Class I Fingerprint Clearance Card
• Pass substance abuse testing
• Provide an updated immunization (IZ) record
• Have a current (within 12 months) Tuberculosis (TB) skin report
