Job Description
An exciting opportunity has arisen for an Assistant Practitioner to join the Eastern Community Older People and Physical Disability Team in Great Yarmouth, covering central Great Yarmouth and the northern villages. The successful candidate will undertake Care Act Assessments, Support Planning, Reviews, and take part in covering the duty system, working collaboratively with partners and stakeholders to promote independence.
