Job Description
As a Clinical Case Manager, you will provide intensive services to families at risk of out-of-home placement, educate them in social-emotional and parenting skills, and work with a partner clinician to provide needed services. You will also maintain case records, participate in treatment plan meetings, and provide liaison contact with collateral resources.
Requirements
- Bachelor's degree in social work, Sociology, Criminal Justice, Psychology, Human Services, or a related field
- Experience working with at-risk youth and families
- Home and community-based experience is a plus
