Graduate of accredited school in Radiologic Technology, ARRT certification in CT, and current IEMA license

Illinois medical license, DEA/Controlled Substance License, and Board Certification/Eligibility
Advanced Practice Provider - Weekend Hours
5+ years of primary care experience, Illinois nursing license, current certifications for APNs and PAs, CPR certification
Primary Care Physician - Internal Medicine - Skokie ACC
Board certification/eligible in Internal Medicine, residency, and medical licensure

ENT Sleep Surgeon
Fellowship trained in Sleep Medicine, IL Medical License, IL DEA License, BC/BE by the American Board of Otolaryngology – Head and Neck Surgery
