LICENSED CLINICAL SOCIAL WORKER - CASE MANAGEMENT

Northwest Specialty Hospital is seeking a team-oriented Licensed Clinical Social Worker to join our Case Management Team. The Licensed Clinical Social Worker is responsible for providing initial evaluations, individual, and group therapy, as well as consultation services and suicide risk analysis.

Requirements

• Provides consultations to clients
• Initial mental health evaluations resulting in treatment plans
• Assessing clients using standardized assessment/screening tools, measures, etc.
• Provide brief, solution focused therapy
• Provide group psychoeducation/treatment related to chronic medical conditions;
• provide information and referral services;
• Care coordination
• Crisis assessments/triage with appropriate interventions
• Provision of other services related to wellness
• Documentation of services provided in accordance with requirements
• Other related activities designed to provide integrated health care services
• Individual psychotherapy with clients
• Documentation and scheduling in electronic health record
