Job Description
Northwest Specialty Hospital is seeking a team-oriented Licensed Clinical Social Worker to join their Case Management Team. The position is responsible for providing initial evaluations, individual and group therapy, and consultation services to patients.
Requirements
- Provides consultations to clients
- Initial mental health evaluations resulting in treatment plans
- Assessing clients using standardized assessment/screening tools, measures, etc.
- Provide brief, solution focused therapy
- Provide group psychoeducation/treatment related to chronic medical conditions
- Provide information and referral services
- Care coordination
- Crisis assessments/triage with appropriate interventions
- Provision of other services related to wellness
- Documentation of services provided in accordance with requirements
- Individual psychotherapy with clients
- Documentation and scheduling in electronic health record
