Job Description
The Manager, Community Wellness Hubs provides leadership, operational oversight, and strategic coordination for Community Wellness Hub services in Thunder Bay. The position oversees interprofessional team members delivering primary care, health promotion, and social prescribing supports to underserved populations.
Requirements
- Master's degree in a healthcare discipline
- Minimum 3-5 years of leadership or management experience in community health or human services
- Knowledge of community-based primary health care and social determinants of health
- Experience working with Indigenous, BIPOC, 2SLGBTQIA+, and culturally diverse populations
- Demonstrated leadership experience within an interprofessional model of care
- Experience with community engagement and partnership development
- Knowledge of data collection, program evaluation, and performance reporting
- Experience using Electronic Medical Records systems
- Valid Ontario Class G driver's license and ability to travel
