Job Description
We are seeking a System Navigator to provide support for clients with complex and multiple needs in the communities served by NorWest Community Health Centres. The position involves assessing and planning with clients, facilitating and advocating for services, and working with an interdisciplinary team.
Requirements
- Honours Bachelor degree in Social Work (HBSW) or Registered Practical Nurse diploma (RPN)
- Minimum one-year work experience
- ASIST Training
- Ability to effectively engage clients
- Demonstrated ability to facilitate and advocate for services
- Ability to work both independently and collaboratively with an interdisciplinary team
- Strong writing and documentation skills
- Strong understanding of the determinants of health framework
- Strong understanding and knowledge of health services and community supports
- Ability to work flexible hours
- Ability to work effectively with marginalized and under serviced population
- Awareness of and sensitivity to health issues
- Awareness and sensitivity to socio-economic and cultural issues
- A current driver’s license and access to a reliable vehicle
